Log in to your Life Time account online or in the app. Submit the cancellation request, or use the contact form. Or cancel in person at your club. Give advance written notice per your General Terms Agreement. Keep the confirmation; expect billing through the notice period.
Advance written notice is required (widely reported as 30 days), so expect at least one more dues charge.
