Contact your home location (the franchise where you joined). Ask how they need the request (in person or in writing). Get the cancellation date and final charge in writing. Book any banked sessions within 60 days of cancelling. Watch your card statement for later charges.
Unused accrued sessions are forfeited if not used within 60 days of cancellation. Your Wellness Agreement is with the franchise, so notice rules can vary by location.
