Contact the spa where you bought the membership. Ask them to schedule your termination and confirm the date. Expect possibly one final payment on the next billing date. Use leftover credits within 60-90 days of your request.
One final membership payment may still be charged. Credits then expire 90 days after your request (60 days if no final payment) unless you're in California, where they don't expire but are redeemed at non-member prices.
